What should I bring to my first appointment?

New Patient Form, Insurance Card(s) and a valid driver's license. Current list of medications you are taking. Any medical records & lab reports you have relating to your visit. You are expected to bring your blood glucose logs, monitor or any lab reports from another MD. Visit with the MA on history taking followed by visit with a provider MD or NP for examination and treatment plan. Referral from your primary care physician. Your insurance company may require the referral to be dated prior to the appointment date.

What if I am running late for my appointment?

If you are on your way but could not make it in time (within a grace period of 30 minutes), contact us. If contacted your appointment could be rescheduled for the same day, else it will be canceled. If you are unable to visit on the appointment date, please notify us 24 hours in advance.

Do you accept my insurance?

We accept a variety of insurance plans. We do not accept Medicaid primary, but do accept as secondary coverage. For more information, please call your insurance company to ensure that we are in their network.

What is the usual length of an appointment?

You should expect a 2-hour visit from check in to check out for first-time patients and at least 1 hour for follow-up patients.
