Clinic Ratings Index 2026
One in ten rated US clinic listings holds a flawless 5.0. Which ten depends far less on the clinic than on what kind of clinic it is: the share runs from 39.9% in chiropractic down to 3.2% in urgent care.

Perfect ratings cluster hard by specialty
Across every rated listing, 6,446 carry an unblemished 5.0. Split those by specialty and the spread is more than tenfold. Hands-on, repeat-visit practices sit at the top; the places people go when they are acutely unwell sit at the bottom.

What this does not show
A perfect rating is not evidence of anything improper, and this analysis cannot distinguish between the explanations. The most likely one is arithmetic: a clinic with eight happy patients scores 5.0 far more easily than one with eight hundred, and small practices dominate the specialties at the top of that chart. Review solicitation habits differ by specialty too, and a physical therapist who sees the same patient twelve times has more opportunity to ask than an urgent care clinic does.
Controlling for review volume would settle most of that, and we have deliberately not done it: the review counts in this dataset are not reliable enough to divide by, and a weighting built on them would look rigorous while being wrong. The chart shows the distribution. The causes stay open.
A quarter of listings have no rating at all
24,497 of 92,083 listings, 26.6%, have no public rating we could find on any platform. For a patient who chooses a clinic by reading reviews, a quarter of the market is effectively invisible, and that is not the same population as the badly-reviewed quarter.
By state
Rhode Island leads at 13.9% of rated listings at 5.0; New Mexico is lowest at 6.1%. The state spread is real but much narrower than the specialty spread, which is the more interesting result: what a clinic does predicts its rating better than where it is.

Excluded from the ranking for having fewer than 100 rated listings, where a percentage would describe a handful of clinics: District of Columbia (35 rated).
Method and limitations
Every listing in the Clinic Directory database was counted as of 2026-08-18: 92,083 clinics across all 50 US states, Washington DC, and 25 specialties. Ratings are aggregated from public review platforms, normalised to a 0 to 5 scale. “At 5.0” means an aggregate of exactly 5.0. Percentages of rated listings exclude the 26.6% with no rating.
This describes a directory's coverage, not the American clinic population. These listings are assembled by crawling public sources, so a clinic with no web presence is absent from the denominator entirely. This is not a registry and should not be read as one, or compared against provider counts from NPPES or HRSA, which are built very differently. Rating data is only as current as the last time each clinic was indexed.
Insurance acceptance, language and telehealth availability were examined and deliberately excluded. Those fields are unnormalised free text in the underlying data, and any percentage built on them would be precise-looking and false.
