Do I need a referral?
A referral is required for Medicare and Vermont Medicaid patients. For other insurance carriers, obtain a referral from your primary care physician or orthopedist, even if not specifically required.
What should I bring to my first appointment?

How long will my appointment be?
Initial Evaluation: ~1 hour (arrive 15 minutes early). Follow-up visits: ~45 minutes.

What is your cancellation policy?
24-hour notice required; missed or within 24 hours incurs a $75 fee; three no-shows may lead to discontinuation of treatment.
