All students are eligible. We accept Medicaid, commercial insurance, and uninsured students.
Is there a cost to families?
There is no cost to families, even if your child is uninsured.
How to enroll?
Submit a consent form anytime during the school year. To ensure your child is seen at the next clinic, please enroll as soon as possible.
What forms are available for enrollment?
Futures Online Consent Form and a paper form.
Where to return forms?
Return completed paper forms to the school nurse or to the Healthy Futures email: healthyfutures@heartlandhealth.org.
